Twenty-three-year-old sprinter Thando Dlodlo tested positive for testosterone and “related compounds” in April of 2021, at the South African championships which took place in Pretoria. SA Institute for Drug-Free Sport (Saids) confirmed the infraction and ban.
Dlodlo and teammates, Akani Simbine, Tlotliso Gift Leotlela, and Clarence Munyai will lose the gold medals they won competing at the 2021 World Athletics Relays in Chorzów, Poland in May of last year.

What this means is Italy, which won the silver medal will move up to the gold position. Japan and Denmark, move up to silver and bronze.
Dlodlo confessed to taking testosterone and has been banned for four years. The ban was reduced to two and half years retroactively.
